Ervad Brigadier Panthaki will conduct the Muktad Prayers. Each family may submit up to 5 names for inclusion in the prayers. Fill in the form that will be sent by mail and return to Ervad Brigadier Panthaki with a donation of $5.00 per name by August 8, 2009 (phone messages will not be accepted) Checks to be written in the name of ZAMWI and sent to:

Flowers may be offered at the ceremony.
You may bring a framed picture of the departed for the Muktad Table.
Sandalwood and candles will be available on payment.
There will be a Potluck Dinner after the Ceremony.
